About Khaleidoscope Health Care
Founded and incorporated in 1988, Khaleidoscope Health Care is licensed as an ambulatory primary care provider (primary and internal medicine, ob/gyn and pediatric services) and substance abuse treatment provider specializing in addictive services.

Khaleidoscope Health Care provides the medical attention that its patients need and the treatment individuals require in a clean and cheerful atmosphere that is respectful of them as people — not just as patients.

​Khaleidoscope operates from two sites in the middle of a predominately minority community. One is a 1,000 square-foot facility at 127 Lafayette Street (Parkside Medical Center) where affordable primary health care is offered. The other is a full-service ambulatory health center at 75 Harrison Avenue that specializes in addictive care treatment.

In 2003, Khaleidoscope Health Care earned the prestigious accreditation from the Joint Commission Accreditation for Healthcare Organizations (JCAHO). This accreditation is awarded to health care organizations providing quality patient care reflecting changes in health-care practices and current health-care delivery.
